Definition
A standard lipid panel measures four values: total cholesterol, LDL-cholesterol (low-density lipoprotein), HDL-cholesterol (high-density lipoprotein), and triglycerides. Total cholesterol alone is a poor cardiovascular risk predictor — it includes both atherogenic (LDL, VLDL) and protective (HDL) fractions. LDL-C is typically calculated using the Friedewald equation, which underestimates true LDL in patients with elevated triglycerides (a common finding in insulin resistance). Triglycerides measured fasting reflect recent carbohydrate load and are one of the most sensitive and practical markers of insulin resistance available on a standard panel. HDL is the reverse-cholesterol-transport lipoprotein; its decline during menopause is one of the mechanisms underlying postmenopausal cardiovascular risk acceleration. The triglyceride-to-HDL ratio (Trig ÷ HDL) below 2.0 is associated with healthy insulin sensitivity; above 3.0 suggests likely small dense LDL elevation and insulin resistance even when LDL-C appears normal.
