Definition
The thyroid gland produces primarily T4, an inactive prohormone that must be converted to T3 — the biologically active thyroid hormone — in peripheral tissues. This conversion depends on selenium, zinc, iron, and a functioning liver and gut. The entire system is regulated by TSH (thyroid-stimulating hormone) from the pituitary gland. Thyroid dysfunction is classified as hypothyroidism (underproduction), hyperthyroidism (overproduction), or subclinical forms where TSH is mildly abnormal but T3/T4 are in range. Hypothyroidism — the most common form in women — produces fatigue, weight gain, cold intolerance, constipation, brain fog, dry skin, hair loss, and depression. Subclinical hypothyroidism can produce similar symptoms without meeting the threshold for formal diagnosis. Full thyroid evaluation requires TSH, free T3, free T4, and antibody testing — not TSH alone.
